I give permission to the LHS summer school program to take whatever emergency measures are judged necessary for the care and protection of my child while under their supervision. In case of a medical emergency, I understand that my child will be transported to an appropriate medical facility by the school authority. While taking this action, the parents/guardians are notified. I understand that any expenses incurred will be borne by the child’s family.

I give permission to the LHS summer school program to publish any or all pictures of my child taken during the duration of this program.

I give permission for my child, to participate in any outings during the course of the LHS summer school program.

Payment submitted with the registration form
A non-refundable registration and program fee is due with the completed registration form. Space is not held or secured until a completed registration form and full payment for the program is received.
FEES FOR THE SUMMER SCHOOL
N4,000.00 per day per child
N16,000.00 per week per child
N60,000.00 for 4 weeks per child
Early bird payment – Pay on or before 5th July, 2019
N48,500 per child for the 4 weeks
Group discount of 4 children or more – 10% off per child
AVAILABLE:
Healthy Lunch - N1,000.00 per day
After School Care (3pm – 6pm) – N1,000.00 per day
